PIN Blocked call provider and PUK codes

Not sure what your PIN code is, or did you forget your PIN code on your SIM enabled phone, and now it says PUK blocked?
It's a common question we recieve at FixYa.com and WebAnswers.com and it's frustrating not to be able to use your phone.
A SIM PIN is a Subscriber Identification Module Personal Identification Number, and allows you to keep the content of your SIM private. This is different than a lock code which protects your phone from being used in the event that it is lost or stolen.
If you get the message PIN block, be very careful; if you enter it incorrectly three times, you'll get another status message, PUK (PIN Unblocking Key) blocked. If you haven't changed your PIN code, the default is 0000 (four zeros). If you have changed your PIN code, simply enter the wrong code three times to get to the PUK blocked code.
Your PUK unblocking code is the next level of protection in the event that you enter the PIN wrong; and requires the intervention of customer care to resolve. The security policy varies by cell phone provider, make sure that you're authorized on the account prior to calling.
Once you're given the PUK code, the phone should prompt you to change the PIN code; if you don't plan on using the PIN code, reset it to four zeros to eliminate confusion (and forgetfulness!) in the future.

Actually, the PUK number doesn't have anything to do with your cell phone number, but the SIM ID on the phone. You'll have to call your provider to get it.

Marty - a couple of questions:
    1) What does the phone say exactly?
    2) What events led to this phone getting this error message?
The possibility exists that you may have to get a starter kit so you can get another SIM card - then call Boost and get them to change the SIM ID to the one you just purchased.
